Therapist Opportunities
We require the following items when contracting with a therapist:
- Therapist License
- CPR Card
- TB Test/results
- Drivers License
- Resume with References
- Social Security
- Insurance- (see below)
(1) Professional liability insurance covering itself and/or employees for the professional services to be rendered under this Agreement in the minimum amount of five-hundred-thousand ($500,000) dollars per occurrence and one million ($1,000,000) dollars annual aggregate.(2) Comprehensive general liability insurance covering itself and/or employees against property damage, bodily injury or death in a minimum amount of one-hundred-thousand ($100,000) dollars per occurrence and five-hundred-thousand ($500,000) dollars annual aggregate.
